Discover Why Statins Often Lead to Muscle Pain: Causes and Solutions

bigstock-Adult-Female-With-Muscle-Pain-428544833-scaled Lifestyle

Statins may cause muscle pain by affecting muscle cell energy. Tell your doctor, and do not stop them on your own.

Why

Statins may affect muscle cell energy production (coenzyme Q10 and mitochondria). Genetic factors, dose and interacting drugs play a role.

How common

Muscle symptoms occur in about 5% to 10% of users in practice.

Serious

Rhabdomyolysis is rare (under 0.1%).

What to do

Tell your doctor. Don’t stop on your own.

Options

Why does statin cause muscle pain: main illustration

Lower the dose, switch statins or take it on alternate days.

Emergency

Severe muscle pain with dark urine.

Prevalence and mechanisms

Statin-associated muscle symptoms are reported by about 5 to 10 percent of patients in observational studies, though blinded trials suggest the true rate caused by statins is lower. The 2020 SAMSON trial in the UK found that about 90 percent of symptoms reported on statins were also reported on placebo, a pattern called the nocebo effect.

Why does statin cause muscle pain: visual explanation
  • Proposed mechanisms include reduced coenzyme Q10, effects on muscle energy production and genetic variants such as SLCO1B1.
  • Risk factors include older age, female sex, low body mass, hypothyroidism, kidney disease and interacting drugs such as clarithromycin.
  • Rhabdomyolysis, severe muscle breakdown, is rare, at roughly 1 per 10,000 patient years.

What to do

Do not stop taking statins on your own. Doctors may check creatine kinase and thyroid levels, lower the dose, switch statins (rosuvastatin, pravastatin), try every-other-day dosing or add non-statin drugs like ezetimibe. Severe muscle pain with weakness and dark urine needs urgent care, since it can indicate rhabdomyolysis.

Prevalence and mechanisms

Muscle symptoms are the most common reason people stop statins. Randomized trials find that muscle pain is reported by about 1 to 5 percent more people on statins than on placebo, while observational studies report higher rates of 10 to 25 percent. A 2022 Lancet meta analysis of 19 blinded trials found that only about 1 in 15 muscle complaints in people on statins were caused by the drug. Proposed mechanisms include reduced coenzyme Q10 and effects on muscle cell energy.

What to do

Do not stop the drug without talking to your doctor. Options include a lower dose, a different statin such as rosuvastatin or pravastatin, alternate day dosing, or non statin drugs such as ezetimibe. Severe weakness with dark urine can signal rhabdomyolysis, a rare emergency. Doctors may check creatine kinase, thyroid and vitamin D.

Statin types and risk factors

Risk of muscle symptoms rises with higher doses, age over 80, small body frame, kidney or liver disease, hypothyroidism, vitamin D deficiency, alcohol and drug interactions with clarithromycin, cyclosporine, gemfibrozil and grapefruit juice in large amounts. Simvastatin 80 mg carries the highest risk and the FDA restricted it in 2011.

Management and monitoring

Doctors may check creatine kinase and thyroid, and may pause the statin for 2 to 4 weeks then rechallenge. Alternatives include pravastatin, fluvastatin, rosuvastatin at low dose, pitavastatin, ezetimibe, bempedoic acid and PCSK9 inhibitors. Statins cut major cardiovascular events by about 25 percent for each 39 mg/dL LDL reduction. Rhabdomyolysis occurs in about 1 per 10,000 patient years.
