Fluttering in the ear is often a small ear muscle spasm or Eustachian tube movement, usually harmless. See a doctor if it persists.

Cause

Usually spasms of tiny middle ear muscles (tensor tympani or stapedius), or the Eustachian tube opening.

Triggers

Stress, loud noise, caffeine and fatigue.

Other causes

Palatal myoclonus and TMJ.

Is it harmful

Usually harmless and short lived.

What helps

Reduce stress, caffeine and noise.

If it lasts weeks, or comes with hearing loss or dizziness.

Cause of ear fluttering

A fluttering or thumping sensation in the ear is often caused by tiny muscle spasms in the middle ear. The tensor tympani muscle and the stapedius muscle contract involuntarily, producing a flutter. This is sometimes called middle ear myoclonus, or tonic tympani syndrome.

What causes fluttering in ear: visual explanation
  • Triggers include stress, anxiety, loud noise exposure, caffeine, lack of sleep and TMJ problems.
  • Eustachian tube dysfunction and fluid in the ear can cause similar sensations.
  • Palatal myoclonus (rhythmic contraction of the soft palate) causes clicking sounds in the ear.

Is it harmful

Usually it is benign and goes away on its own. Reduce stress, limit caffeine and protect ears from loud noise. See an ENT doctor if the flutter is persistent, associated with hearing loss, dizziness, pain or discharge. Treatment can include muscle relaxants, botulinum toxin or surgery in rare, severe cases. Pulsing in rhythm with the heartbeat is a different issue, called pulsatile tinnitus.

Cause of ear fluttering

Fluttering in the ear can be from middle ear muscle spasms (tensor tympani or stapedius), Eustachian tube dysfunction or tinnitus. Stress and caffeine can trigger.

What causes ear fluttering

A fluttering feeling in the ear is often a muscle spasm. The tensor tympani and stapedius muscles in the middle ear can contract involuntarily, a condition called middle ear myoclonus, producing a thumping or fluttering sound. Eustachian tube dysfunction, palatal myoclonus, anxiety, stress, caffeine, jaw problems (TMJ) and tinnitus are other causes.

Management

Most cases are harmless and resolve. Reduce stress, caffeine and loud noise exposure. See an ear nose and throat doctor if it persists beyond a few weeks, or if it comes with hearing loss, dizziness, pain or discharge. Treatments include addressing Eustachian tube problems, muscle relaxants, botulinum toxin for severe cases and rarely surgery cutting the tendons. An audiogram and tympanometry help evaluate.
